Third-body damage of prosthetic heads in total hip replacement: combined experimental and computational study

چکیده
Introduction. In artificial hip joints, the roughening of the femoral head has been associated to higher wear rates of the polymeric cup, eventually leading to osteolysis and implant failure. The increase of the femoral head roughness is strongly influenced by the presence of abrasive particulate entrapped between the articulating surfaces (Bauer et al., 1994 and Raimondi et al., 2000). This damaging mechanism has been simulated in controlled in vitro wear experiments (Que et al. 1999). The mechanical parameters influencing the scratch formation are still to be determined. The aim of the present study is to evaluate the dependence of such damage on the geometry of the hard particles entrapped in the joint, with reference to the commonly implanted UHMWPE/chrome-cobalt coupling. To this purpose, a series of chrome-cobalt femoral heads, retrieved at revision surgery after a period of in situ functioning, has been investigated for the occurrence of third-body damage. The damage observed on the surfaces has been divided into four typologies, on the basis of the defect morphology. A FE model has been built, which simulates the local behaviour of the two coupled bearing surfaces, in the presence of those entrapped hard particles. An index of induced head damage has been defined on the basis of the computational results and discussed in relation to the experimental findings.

A Case Report-Neck fracture of a cemented stainless-steel femoral modular stem, 8 years after Total Hip Arthroplasty
Total Hip Replacement is a method of prosthetic reconstruction of the acetabulum and proximal femur following hip joint destruction. Prosthetic fractures have been reported after primary surgery ; with majority of them at the prosthetic stem. Few had fracture through neck of prosthetic stem. Here, we report a rare case of fracture neck of prosthetic stem within 10 year of implantation in a pati...
متن کاملBone density study of the proximal femur after hip arthroplasty with porous-coated implants
Progressive loss of bone mineral density around the femora! component of total hip replacement continues to pose a threat to long term prosthetic survival. A linear study was undertaken to measure bone mineral density on a monthly basis following total hip arthroplasty in 11 male patients. The opposite femur was used as the control measurement. Bone mineral density was unchanged at two mo...
متن کاملBilateral Staged Total Hip Replacement and the Natural Progress of an Untreated Case of Developmental Dysplasia (Dislocation) of the Hip: A Clinical Case Report by the Surgeon and the Patient
The natural history of an untreated case of a Developmental Dysplasia (Dislocation) of the Hip (DDH) associated with multiple congenital abnormalities is reported in a 55-years-old man. The patient’s complaints and the varieties of the typical manifestations emerged in other parts of the body throughout the life are reviewed and discussed as comorbidities of a dysplastic condition. Two-stage bi...
متن کاملMicrobiological Culture Findings of the Femoral Heads as a Prognostic Factor in the Total Hip Replacement Surgery
BACKGROUND In primary total hip replacements (THRs), the dissected femoral heads (FHs) are commonly used to make the bone-chips for the reconstruction in the orthopaedic surgery. The donated FHs are routinely microbiologically cultured to identify and contaminated FHs are discarded. This study examines whether a positive FH culture predicts an infection and prosthetic failure after primary THR....
متن کاملAttempt to classify patients with arthritis of the hip suitable for prosthetic replacement, and their femoral heads.
Using clinical, radiological, and morbid anatomical data about patients with arthritis of the hip and their femoral heads resected during total hip replacement, an attempt has been made first, to classify patients and femoral heads by a cluster analysis technique, and secondly and more realistically, to use existing clinical and radiological classification as a basis for seeking discriminating ...
